The Laredo Vital Statistics office hours of operation are Monday through Friday 8:00 a.m. – 5:00 p.m. All Birth and Death records requested are issued within the same day.

Requirements to obtain a Birth or Death Certificate
You must be a qualified applicant · Qualified applicant is the registrant (person on record) · An immediate family member (parents, grandparents, or siblings) · Spouses (a marriage license will be required) · Legal Guardian (proper documentation is required) · Legal Representative (proper documentation is required) Proper identification is required: · Current State-issued driver’s license · State/city/county ID card · Government employment badge or card · Prison ID · Military ID · Passport (any country) OR Two official documents that bear the signature of the applicant (e.g. Social Security card, Voter’s Registration card, Hunting/fishing license, Library card, Student ID, Income Tax Forms 1040EZ, 1040A, or 1040)

Apply by mail: Mail in completed application for request of a Birth or Death Record APPLICATON FOR MAIL IN REQUEST - Download Application Copy of current Identification for applicant is needed. Acceptable types of identification are listed above. Send a money order for the type of certified copy being requested plus $5.42 for mailing of certificate by certified mail.

Amendment to a Birth or Death Certificate
The most common change made to a birth certificate is the addition of the father’s name. This change to a certificate can be made by submitting the Application for a New Birth Certificate Based on Parentage Form (VS-166) (http://www.dshs.state.tx.us/vs/reqproc/amendparentage.shtm)signed by both parents in the presence of a notary public with one of the following documents: · Certified copy of a marriage license between the mother and the biological father · Properly completed State of Texas Acknowledgement of Paternity Form (VS-159.1) · Certified copy of a court decree establishing parentage between the child and the biological father
