Overview

Our commitment is to enhance community-based services for Children and Youth with Special Health Care Needs (CYSHCN) and to improve the systems of care they rely on. Our clientele includes beneficiaries of health care benefits provided through the Health and Human Services Commission’s CSHCN Services Program.

Our CYSHCN program collaborates with multiple stakeholders, including:

  • Families.
  • Community organizations.
  • Universities.
  • National and state technical assistance centers.
  • Other state agencies and programs.

CSHCN Systems Development Group Areas of Focus:

  1. Transition:
    • Adult care transition services and support.
    • Transition is a dynamic, lifelong process that addresses youth and family's individual concerns as the youth transitions from childhood to adulthood.
  2. Medical Home:
    • A medical home offers comprehensive primary care, facilitating partnerships between patients, physicians, and families.
    • Key characteristics of care in a medical home include comprehensive, coordinated, continuous, accessible, family-centered, culturally competent, and compassionate care.
  3. Community Inclusion:
    • FSCR – CYSHCN strives for accessible and welcoming Laredo Community and promotes inclusive activities and services.

      • Emergency Preparedness
      • Education
      • Awareness

 

FSCR helps refer families of children and youth with special health care needs (CYSHCN) to relevant state programs. One such program is the Children with Special Health Care Needs Services Program, which offers assistance to:

  • People aged 20 and below with special health care needs.
  • People of any age with cystic fibrosis.

The main objective of the program is to enhance the health, well-being, and overall quality of life for these individuals. It achieves this by implementing family-centered and community-based strategies, ensuring that services provided honor and respect the cultural beliefs, traditions, and values of the families involved.

To qualify for services, all provisions must be deemed medically necessary. The program covers a wide range of services, including:

  • Medical care
  • Mental health support
  • Dental services
  • Vision care
  • Specialized medical equipment and supplies
  • Family support services
  • Community resources
  • Case management

 

By offering these comprehensive services, the program aims to address the diverse needs of CYSHCN and individuals with cystic fibrosis, promoting their overall well-being and quality of life.
